1. Print or Save a copy of this page in your computer's notepad or other word/office program. Open office is free online.

2. Print out or Fill out this form in notepad or other word/office program.  Write the name of your organization that gets % of the total sales prices as a donation from Jeffrey. (The more you order the more they get!)

3. List item(s) title(s)/description(s), print size(s), listed price(s), and finally then the total price paid. MD residents add 6% Maryland sales tax.  

4. Clearly print your name, address, telephone number, and an email contact so we can be sure your group gets their gift money.

5. Send a fully completed copy of this page along with a check or money order made out to Jeffrey M. Ferrell Photographer to his contact address  (web orders do not qualify for the charity donations, and are not easily tracked at this time) Any questions just email Jeffrey or call him. 

6. Double check your order, mail and enjoy your new art!

7. Jeffrey the artist, will personally mail you a thank you confirmation after receiving your order along with a free collectable bonus print.

Prints are custom made for each order, no prints are kept in inventory. Delivery time is three to six weeks (and after checks clear), unless other arrangements are made. Many times prints will be freely delivered to the organization's headquarters for pick up. Sometimes Jeffrey may surprise a client and personally deliver the art. Distant personal art delivery by Jeffrey is additional-call first.

Your group may also qualify to get the same cash producing benefits as the above offer! 

A very limited number of groups could receive 100% of all proceeds, but they must prove their extreme value to a uncommonly worthy cause that Jeffrey trusts, believes in and establishes a long term mutually valuable partnership with.
